Parent Authorization

If my child commences music lessons with BRIGHT MINDS,

I agree:

My child intends to continue lessons for the full academic year. Notice of withdrawal must be received in writing four weeks prior to discontinuing.
Enrolment continues from year to year unless notification of withdrawal is given as stated above.
If you know your child won’t be able to make a lesson, you must contact the tutor directly at least 24 hours prior to the lesson. In these cases the lesson will be rescheduled. Failure to do so will result in you being charged for the missed lesson. In the case of illness, kindly notify your tutor directly before 8:30am on the day of the lesson. In this case, all efforts to make-up the lesson shall be made.
Lessons must be paid for in advance. Lessons are billed according to periods of no longer than 10 weeks as decided by BRIGHT MINDS, taking in to consideration the various school timetables of our students. A minimum of THREE weeks lessons must be paid in advance.
Maintain communication with the tutor to ensure the best possible education outcomes for the student.
Make sure that the student attends and is collected from the lesson in a punctual manner.

The current fees for individual lessons in 2016 are as follows:

STUDENT COMMITMENT

I choose to take part in the BRIGHT MINDS music program for 2016. I am committed to the following:
To practice my instrument for the time decided on with my teacher.
To look after my instrument, whether hired from the school or my own
To remember to keep a log of my personal practice and bring this to my lesson.
